60 minute in-person comprehensive evaluation covering medical history, psychiatric history, social history and family history. We will determine a working diagnosis and treatment plan.

Follow Up
30 minute in-person or televideo appointments to monitor progress, make medication adjustments, and/or provide counseling

Second Opinion/Other Consultation
Comprehensive evaluation to provide a professional opinion without establishing a treatment relationship. Includes consultation for expert witness and industry guidance.
$500 per hour
I am an out of network provider. If you have out of network benefits, our office can submit reimbursement claims on your behalf which typically cover 50-80% of costs. We can check your out of network benefits if you provide us with your insurance information. HSA/FSA, Venmo, Zelle, credit card, cash and credit are accepted payment options due on the date of service.
