Cancellation Policy

To maintain an efficient schedule and accommodate all our patients, we require at least 48 hour notice if you need to cancel or reschedule your appointment.

Failure to provide the required notice or not showing up for your appointment disrupts our ability to provide good care to you and our other patients. In such cases, a fee at a rate of $100 per 30 minutes of reserved appointment time will be charged. This fee is not covered by insurance and is the patient's responsibility. 

Because we understand that unforeseen circumstances do happen, the first late cancellation is waived for established patients. 

Consistent late cancellations or no-shows may result in potential discharge from our practice. 

A $200 deposit is required to schedule the initial appointment. The deposit is applied to the cost of the appointment and is refundable with 48 hours notice prior to the scheduled appointment. 

Refill Policy

To maintain highest standards of care and safety monitoring, refills of prescribed medications are contingent upon compliance with adequate follow up. Patients who have not been seen for over 6 months will not be prescribed refills and will be discharged from the practice.

All electronic communications utilized by Concierge Psychiatry are HIPAA compliant.

Information about patient care, diagnosis, treatment, etc will not be disclosed without the patient's consent. Exceptions to this include:
